The decision question One sentence describing the decision required.

Facts The figures and observations that have been verified.

Assumptions What we believe provisionally, or what is unconfirmed.

Options Realistic alternatives, including doing nothing.

Comparison criteria Cost, time, impact, risk, and feasibility.

Effects and risks The upside and the downside of each option.

+ For the advanced reader: field-level accuracy
Structured outputs and schema validation are useful when turning text into fields. But
semantic accuracy at the field level must be tested, not only the correctness of the
structure. A system filling the “amount” field with a readable value does not mean it
extracted the right amount. Build a representative test set, record accuracy for each field,
and set a threshold for human escalation.

What to notice: Each stage has a gate. Skipping a gate does not save time; it moves the
correction to the point where it costs most.
