Training activities:

• Using Access MS, create a database named (Database Patients) Then design the following
table and save it on the desktop under the name (Patients) as shown:

• Make the patient number field of type (number) and make it the primary key of the
table/Choose one of the fields as the primary key of the table.

The field size in the patient name is set to 25, which is a required field
• The size of the field in the number of revisions is a long integer.
• Make the validation rule for the number of revisions greater than or equal to 1, and make

the message text “Re-enter the number of revisions.”
• Format the date of birth so that it is date-long.
• The date the file was opened should be short.
• Make the validation rule for the phone number greater than or equal to 6,000,000, and

make the message text “Re-enter the phone number.”
• Make the data for the “Having health insurance” column type (yes/no) and format it so that

it is a check box
• Make the validation rule for the treatment cost field not less than or equal to 100, and make

the message text “Re-enter treatment cost.”
• Make the number of decimal places for the Treatment Cost field equal to 3 places.
• Add the following description to the “Treatment Cost” field: “The total cost of treating the

patient.”
• Add the currency symbol SAR to the treatment cost (if it does not exist, choose the Euro).
• Save the table as “Patients”.
• Fill in the data as shown.
• Create a query for patients whose treatment cost is less than or equal to 2000, where the

patient's name, address, and treatment cost appear, and name it as Cost.
