● Remember: Section summary
           • Key idea: Verifying media rests on the source, the context, the channel, and the
procedure, not on visual or vocal realism.
           • Practical decision: Stop the urgency and verify out of band before any sensitive action.
           • Mistake to avoid: Hunting for small technical flaws and treating their absence as proof
           of authenticity.

together with what it does not establish.

4.10 Document: A Light Record That Preserves Responsibility and Learning
◆ Key idea: documentation is not copying the whole conversation.
The purpose of documentation is that another person can understand what was used,
what the decision relied on, and what the human changed. It may be a seven-field record
for a medium task, or a fuller file for a high-impact use. Do not accumulate sensitive
conversations without need in the name of transparency.

Begin by recording the purpose, the tool or model where it is known, the date, a description
of the material entered without repeating sensitive data, the sources used, the most

important human changes, and the name of the reviewer or decision owner. This is usually
enough to reconstruct the logic of the work.

Record the assumptions clearly. If sales data for a particular month was unavailable and the
team used an estimate, that must not disappear inside confident final wording. A

documented assumption can be revisited later; a hidden assumption turns into a fact that
         repeats itself.

Keep versions when a document changes across rounds. Name the versions by date or
number, and record what changed and why. Do not rely on a long conversation log to find

the final decision; create a separate decision summary.
